1、持有中華民國護照之國民，因遺失護照至警察單位辦理遺失證明。 2、本項作業由當事人親自身分證明文件，至各分局核對身分後，即可由受理人員協助線上登打辦理。 3、申請人務須注意，依現行「護照條例施行細則」第39條第1項規定：「護照經申報遺失後尋獲者，該護照仍視為遺失」，亦即護照經申報遺失後，無論是否立即尋獲，依現行規定均須重新申請護照，不得申請撤案，該規定業自100年5月22日起正式實施。 a)Citizens of the Republic of China with passports may go to a police location to apply for proof for losing a passport b)The principal of this application will take their identity papers to a police station for verification. Then the duty officer may enter data online. c)The applicant should notice that Article 39 (1) of Enforcement Rules of Passport Act stipulates, “A passport that is found after it has been declared lost is still considered lost”. In other words, once a passport has been declared lost, the owner must apply for a new passport regardless whether that passport is later recovered or not. The declaration for lost passport cannot be revoked. This stipulation has been in effect since May 22, 2011.

☆申報遺失後再尋獲者，亦視為遺失： 一、國內遺失不得撤銷： 依現行「護照條例施行細則」第39條第1項規定：「護照經申報遺失後尋獲者，該護照仍視為遺失」，亦即護照經申報遺失後，無論是否立即尋獲，依現行規定均須重新申請護照，不得申請撤案，該規定業自100年5月22日起正式實施。 二、國外遺失不得撤銷： 因未持護照入境視為違規入境，各機場、港口之海關已連線通報遺失記錄，故倘若護照尋獲無法辦理尋獲撤銷。 a) Lost domestically: The principal party takes their identity papers to a police station, fills out in duplicate a declaration of losing the passport of the Republic of China. The prescient enters data and waits for online return of passport number and issuance date, which the duty officer enters on the form, affixes a seal specially for passport business and the job seal of the officer, fills in the document number, and gives a copy to the applicant while keeping the other copy for the files of the criminal section at the prescient. b) Lost abroad: 1. If a proof from a foreign police station (excluding China, Hong Kong, and Macau) and a proof of entry into the country are available, the applicant may go directly to Bureau of Consular Affairs, Ministry of Foreign Affairs to apply. 2. If a proof from a foreign police station is not available, the applicant should take their proof of entry into the country to make the declaration at a police prescient.
